Our office hours are Monday through Thursday 7AM – 4PM. We do make every effort to schedule your appointment at a time that is most convenient for you. We will provide you with a courtesy call prior to the date of your scheduled appointment. We do recommend scheduling your next appointment at the time of your current appointment. We do accept new patients, so please call and check our schedule availability.
Your First Visit
It is important that our dentists and staff be notified immediately of any medical conditions that may concern you or about any medications you may be taking. If it is your first visit with us we request that you fill out our Onlne Patient Registration Form (http://mangandental.com/newpatient form/). If you have any questions along the way, please feel free to call us and we will walk through those with you.
Once you have completed the online form be sure to click submit to get the form to us. If you download the form as a PDF please complete it and bring it with you on your first visit. If you have dental insurance that you would like for us to file, please bring your card with you.
When you arrive at our practice you will be greeted by one of our team members, and given a tour of the office. Or you can take that tour online at https://goo.gl/maps/WJW8dkB3XQS2 so that you are immediately comfortable with our facility.
Your initial appointment will consist of a comprehensive oral evaluation, cancer oral screening, periodontal records, and intraoral pictures. On your second visit, you will meet with your designated hygienist for recommend hygiene treatment. A personalized treatment plan will be presented and all of your treatment options will be explained.
In the case of an emergency the treatment can often be completed in one day.
When you are considering a specific kind of procedure with our office you will be asked to complete an informed consent form. These give you the good, bad and ugly details of the treatment you are about to receive. They are a balanced story, but do sometimes over emphasize the negative (as do all informed consent forms) but we want you to be sure and know all of the details of a particular treatment. All payments are due in full at the time services are rendered. We accept all major credit cards, insurance plans and gladly accept personal checks.

Please understand that the contract regarding your dental benefits is among you, your employer, and your insurance company. Your obligation to our practice is to pay for the treatment, regardless of the amount that may or may not be reimbursed by your insurance company.
Our office will accept payment directly from your insurance company with the following provisions. Although we are willing to submit insurance forms and claims on your behalf, provide any documentation to you or your insurance company to sort out any questions or confusion that may arise and will cooperate with the regulations and requests of your insurance company. We do not accept responsibility for the outcome of the transaction. Completing insurance forms is a courtesy we extend to you in an effort to maximize your insurance reimbursement. This does not eliminate your financial obligation to us for your treatment.
If applicable, you will be required to sign document(s) authorizing your insurance company to make payments directly to our office. We make no guarantee that your insurance company will pay for any treatment you receive from this practice, or what amount it may pay. We perform routine insurance billing procedures upon verification of your coverage. If your claim is denied, or paid at a lesser amount than anticipated, you will be responsible for paying the full balance at that time.
We will not enter into a dispute with your insurance company over any claim. It is ultimately your responsibility to resolve any type of dispute over payments made or not made by your insurance company.
